Working in a smaller community hospital with limited resources can present unique challenges for the case management department. With a focus on providing optimal patient care and ensuring efficient utilization of resources, the daily workflow of the department is crucial in maintaining a smooth operation. Let's look into my typical day as a nurse case manager.
The case management department in our hospital consists of a team of dedicated professionals, including utilization review nurses, nurse case managers, and social workers. We work closely together, sharing a secondary office with the palliative care and hospice care teams. This collaborative setup allows for seamless communication and coordination of patient care.
The early morning routine in this role requires various tasks and responsibilities. This includes receiving your assignment for the day and creating a list of tasks that need to be completed. These tasks can range from discharge planning assessments to progress notes, as well as follow-ups with rehabilitation services, home health, or equipment companies. Collaboration with other services and the physician is also a crucial part of the job. The key to managing these tasks effectively is prioritizing based on the urgency and time sensitivity of each activity. By doing so, you can ensure that all necessary tasks are completed in a timely manner and that the needs of the patients are met efficiently.
Some days can be quite relaxed and easygoing. When you have a population with minimal needs, it gives you the opportunity to focus more on social determinants and aspects of care that can ultimately prevent future hospitalizations and enhance the overall health of the patients. However, there are also days that are incredibly hectic and overwhelming. These are the days when you find yourself dealing with insurance companies, coordinating with physicians, managing paperwork, and handling various tasks that demand immediate attention. It can be chaotic and demanding, leaving little room for anything else. The stark contrast between these two types of days in the healthcare profession can make the job both rewarding and challenging.
